Registration:
Please make sure to bring your insurance card and co-payment with you when you come for your appointment. You'll need to arrive at the main entrance to complete the registration process if your appointment is for medical clinic, behavioral health, laboratory, wound/foot care, radiology, or surgical procedures.
However, if you're coming for specialty services, physical therapy, occupational therapy, or speech therapy, please arrive at the MB1 entrance located on the southwest side of the main campus to complete your registration process.
Interpreters:
If you or a family member does not speak English and would like an interpreter, please contact your nurse or a social worker.

Parking:
Parking for patients and visitors is available in the lots directly north and south of the hospital. Handicap parking is available near the entrances. Patients and visitors are asked not to park in the reserved areas or certain designated areas.
Personal Items:
Please inform your care team if you wear contact lenses, dentures, or prostheses. Hearing aids, eyeglasses, dentures, and contact lenses should be kept in containers when not used. You are welcome to bring your hygiene items from home. The hospital provides a toothbrush, toothpaste, comb, and tissues. Please bring your razor. Personal electrical equipment must be inspected and tagged by maintenance before use. Inspection is optional for equipment plugged in for a short time, such as razors, hair dryers, curling irons, etc. No heating pads or electric blankets are permitted.

Health Insurance and Billing:
If you have health insurance, we will submit your bills to your insurance company. Your policy is a contract between you and your carrier; you will be responsible for final payment of your bill. If you do not have health insurance, a financial counselor is available to discuss financial arrangements with you, and will help you in applying for assistance programs, if needed. If you have any questions about your bill, please call (712) 737-5200 and ask for a financial counselor.
Durable Power of Attorney for Health Care: (DPAHC):
A DPAHC is a legal document that allows you (the “principal”) to appoint another person (the “attorney-in-fact” or “agent”) to make medical decisions for you if you should become temporarily or permanently unable to make those decisions yourself. The person you choose as your attorney-in-fact does not have to be a lawyer. Ask a social worker for assistance. Notary service can be requested at no charge by contacting your nurse, Administration at extension 5270, or the Business Office at extension 5251.
Advanced Directives:
An advance directive allows you to let others know your choices for healthcare or to name someone to make those choices for you if you are not able to speak for yourself. You can say “yes” or “no” to life-prolonging treatments. For additional information or to update your advance directive, please ask to speak with a social worker or click the link below.
