Care Coordinator - Hospital

The Care Coordinator is responsible for the appropriate use of inpatient services, through evaluation of admissions, use of ancillary services, and/or length of hospital stay. He/she will be responsible for assessing, coordinating, and implementing the discharge needs of the hospitalized patient.

Education Requirements:

RN. Acute care nursing background preferred and/or case management experience.

Job Schedule:

Days may vary. Fill-in for vacations and other absences/requests off by the other Care Coordinators.
