Bilingual Family Support Specialist (Community Health Parters)

Join the Community Health Partners (CHP) team as a Bilingual Family Support & Community Resource Specialist and help strengthen families through early childhood support, health education, and home-based services.

In this role, you’ll work closely with families enrolled in the Healthy Families Program, providing personalized support that promotes healthy parent-child relationships, child development, and access to community resources. Experience Requirements

Experience working with children and families, valid driver’s license and reliable phone access, strong interpersonal and organizational skills, cultural competence and openness to reflection, knowledge of early childhood development, trauma-informed care, and infant mental health.


Responsibilities

Key Responsibilities:

  • Deliver home-based support services using a strengths-based, trauma-informed approach
  • Coach and educate parents to build strong bonds with their children
  • Monitor child development and identify family needs
  • Connect families to local community resources
  • Maintain accurate, timely documentation
  • Communicate respectfully with families, colleagues, and community partners
  • Collaborate with healthcare providers and agencies
  • Pursue professional growth through training and supervision
  • Work flexible hours, including occasional evenings, to meet family needs


Education Requirements:

Bilingual in English and Spanish. High school diploma or equivalent; Associate or Bachelor’s degree in health, education, or human services preferred.

Job Schedule:

Either Part-Time or Full-Time. 3-5 days/week; Monday - Friday.
