Bilingual Family Support Specialist (Community Health Parters)

The Bilingual Family Support & Community Resource Specialist provides family-centered health and support services, primarily through home visits, to families enrolled in the Healthy Families Program. This includes maternal health services, child development support, and connection to community resources. All responsibilities align with Community Health Partners’ (CHP) mission, vision, and values.

Experience Requirements

QUALIFICATIONS:

Minimum Requirements:

  • High school diploma or equivalent
  • Bilingual in English and Spanish
  • Experience working with families and children
  • Strong interpersonal and relationship-building skills
  • Culturally competent and reflective
  • Valid driver’s license and access to a reliable phone

Preferred Qualifications:

  • Associate or bachelor’s degree in health, education, or human services
  • Knowledge of infant/child development, parenting, and family dynamics
  • Familiarity with trauma-informed care and infant mental health

  • Highly organized, with strong time management and record-keeping skills

Responsibilities

Key Responsibilities:

  • Deliver home-based support services using a strengths-based, trauma-informed approach
  • Coach and educate parents to build strong bonds with their children
  • Monitor child development and identify family needs
  • Connect families to local community resources
  • Maintain accurate, timely documentation
  • Communicate respectfully with families, colleagues, and community partners
  • Collaborate with healthcare providers and agencies
  • Pursue professional growth through training and supervision
  • Work flexible hours, including occasional evenings, to meet family needs


Education Requirements:

Bilingual in English and Spanish. High school diploma or equivalent; Associate or Bachelor’s degree in health, education, or human services preferred.

Job Schedule:

Either Part-Time or Full-Time, up to 32 hours/week. 3-5 days/week; Monday - Friday.
