NOTICE REGARDING BILLING STATEMENTS:
Please check your email for statement notifications, and make sure they are not going into a spam or blocked folder.
If you have a MyChart account with an email address connected to it, you have been automatically enrolled to receive “paperless” billing statements.
(In other words, unless you have opted out, you are no longer receiving your bills from us in the mail.)
If you wish to unenroll/opt-out from paperless billing, please call us at 712-737-5200.

My Chart allows you to:

How do I access My Chart?

This link will take you to Sanford’s registration/access. Once there you will be asked for a one-time ID verification before you are able to login to the My Chart secure services. Note: it may take several days to receive an email confirmation of your account.
Another option is to sign up at our medical clinic registration desk with one of our helpful receptionists. Stop in to the Orange City, Mill Creek, or Hospers Family Practice Clinics or sign up at your next medical appointment. If you want to complete the needed sign-up forms in advance, please download them here, print, complete, and bring into one of our clinics. You will then receive an activation card immediately.
Be sure to write down your Login ID and Password for future access to your online medical records.
