My Chart puts you in control of your medical history. Best of all, it’s available from the convenience of your computer.

My Chart allows you to:
• View your online medical records
• Access your child’s and dependent’s medical records
• Request prescription renewals

• Request an appointment

Note: To request or schedule an appointment with an Orange City Area Health System physician or provider please call our medical clinic at 712-737-2000.


How do I access My Chart?

This link will take you to Sanford’s registration/access. Once there you will be asked for a one-time ID verification before you are able to login to the My Chart secure services. Note: it may take several days to receive an email confirmation of your account.

Another option is to sign up at our medical clinic registration desk with one of our helpful receptionists. Stop in to the Orange City, Mill Creek, or Hospers Family Practice Clinics or sign up at your next medical appointment. If you want to complete the needed sign-up forms in advance, please download them here, print, complete, and bring into one of our clinics. You will then receive an activation card immediately.
